The services are designed to build independent living and tenancy skills and connect people with community-based health care, treatment and employment services. The PSH model has been demonstrated to permanently keep the majority of persons who enter the program from ever becoming homeless again.
Case Management
Our case managers are dedicated professionals who provide personalized support to individuals and families in our programs. Working one-on-one with clients, case managers asses the unique situations, developed tailored action plans and connect clients with the resources they need to achieve stability. With compassion, cultural awareness, and a deep commitment to the mission of disrupting homelessness, Illumination Health + Home case mangers serve as advocates and partners every step of te way, helping clients navigate difficult circumstances and build a brighter future.
Our micro-communities are proven supportive housing units for homeless individuals and families. In this shared housing model, residents have their own private rooms and utilize a common kitchen and living area with other residents. Wraparound services, including case management and counseling, are provided on site.
