We’re Illumination Health + Home.
We exist for one reason: because no one should have to heal on the street. We bring housing and healthcare together to help people stabilize, recover, and rebuild—no matter how far they’ve fallen or how complex their needs. We serve individuals and families facing homelessness, medical vulnerability, and systemic barriers to care. But more than that, we meet people where they are—with dignity, compassion, and support that doesn’t quit.
This is full-circle care. It’s how healing turns into a future.
Homelessness isn’t simple. Neither is the solution.
Illumination Health + Home was built to change that picture. We provide the structure and support people need to move forward—because you can’t treat someone’s illness and send them back to the sidewalk. Because just a bed isn’t housing. And because real recovery takes more than good intentions.
Our mission is simple and urgent: To disrupt the cycle of homelessness by providing healthcare, housing, and hope—together.
Our care doesn’t stop at one service. It surrounds the whole person.
Recuperative Care Centers where healing can actually happen. Emergency and supportive housing that creates real stability. Family-focused programs that break cycles before they repeat. Integrated healthcare and case management that lift people up—not push them through.
Every person’s path is different. But the goal is the same: stability, dignity, and a life rebuilt.

We serve individuals and families experiencing homelessness—many for the first time, many for far too long. Some are leaving hospitals with nowhere to go. Some are living in cars with their children. Others have slipped through every crack in the system.
What they all share is urgent need—and the determination to find a way forward.
